"Have you thought deeply about what we really do as anaesthesiologists? Have you considered the profound impact we have on the lives of patients at their most vulnerable moments? Every day, we engage in our profession, entering the operating room, connecting the induction agent—propofol—to the intravenous stopcock, and inducing a near-death coma in our patients. This state is characterized by profound changes in the brain's electrical activity, making them insensible to any surgical assault.
We then take control of their most basic and fundamental physiological functions, which are evolutionarily conserved across vertebrates.By paralysing the muscles of the body, we inhibit all motion and make spontaneous breathing impossible. Using sophisticated breathing devices, we maintain ventilation and ensure lifesustaining oxygen diffusion. We isolate the cardiovascular system from its normal feedback mechanisms by administering drugs that control blood pressure and heart function. We are not just piloting planes; we are piloting our patients. We use ultrasound guidance to insert needles next to nerves, dissecting human anatomy with invisible sound rather than scalpels.
We inhibit nerve firing with drugs, rendering areas insensitive to pain, so much so that we can perform surgery on a wide-awake patient without them feeling a thing. And then, sensation returns to normal hours later. We are the conductors and coordinators sustaining organ function during heart surgeries, some of which involve cooling the body to numbing temperatures, stopping pumps, and placing patients in a state of suspended animation.
Then, they are warmed and brought back to life, only to awaken hours later as if they had been on a mysterious adventure. What we do can only be described as awe-inspiring. We have the awesome privilege of caring for our patients, imagining the trust they place in us to return them to consciousness in the same or even healthier state than when they entered the process. The skeptic might ask: What’s the big deal? We do this routinely. It is precisely because we have transformed this awesome process into an ultra-safe routine journey through intense research, device development, and practice that makes it all the more marvelous!
So, my message to you, our amazingly talented, superbly trained, empathetic, and kind residents, is simple: When your work feels mundane, boring, or uninspiring, think less about material pursuits and more about the awe and wonder in what we do. It will keep you engaged, curious, and happy. It will enhance your connections with your teams and create greater optimism, joy, and creativity in your work. Continue to find wonder in the world around you. It will keep your relationships with family and friends more satisfying and enhance your overall wellness. For those leaving us, remember your friends, colleagues, mentors, and teachers who have guided you through this transformative period. Most importantly, keep in touch."
